Procedure:

You can apply for your National ID Card at any office of the Ministry of Home Affairs, Immigration, Safety and Security. All applications are sent to the MHAI head office in Windhoek for further processing. Your fingerprints will be matched with those already on record or entered into the National Population Registration System. Your application will then be processed and once issued, your ID card will be forwarded to the office where you applied for you to collect.

Correction / Change
  1. Application for Correction Form (Blue form)
  2. Original ID
  3. Documents to act as proof of requested change [Original (should not be handed in) and copy] – only applies to a Correction. For woman changing to her husband’s surname / or from marriage surname back to maiden name:
  4. Marriage Certificate, Divorce Order or Death Certificate – only applies to a Change

Update of Data
  1. Data update Form (Blue form)
  2. Original Namibian ID (to be handed in), or 
  3. Original or copy of SWA ID (not handed in) 
  4. Original Birth Certificate or Citizenship Certificate (original + copy) or PRP (original + copy)
  5. Fingerprints (taken in office)
  6. ID Photo (taken in office) and Signature

N$ 100.00 (image update)

Duplicate

  1. Application for Duplicate Form (Blue form)
  2. Fingerprints (taken in office)

N$150.00 Incremental for every duplicate issued i.e
1st duplicate = N$150.00
2nd duplicate = N$300.00
3rd duplicate = N$450.00
4th duplicate = N$600.00
5th & all other subsequent duplicates = N$750.00
